Not sure what you mean by how do they calculate VAT? Will assume you mean on the VAT return. On a VAT return, companies state VAT on sales, and VAT on purchases. What is then paid to Customs is the difference between the 2. All that happens is that the VAT will show on the purchases side, not the sales side, and can be reclaimed in full.

Generally vat is charged at every level of value addition. But in pharma it is charged at single stage only.
Now regarding mrp inclusive of vat. Take example. If company is intended to sale x product to end customer @Rs.100/ & if vat is 4% then they will stamp MRP@104 inclusive of vat. So back calculation of 4% will be 3.846% on 104.(100*4/104).
